Medical Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual's health needs. This is provided through advocacy, communication, education, and identification of service resources to promote quality, cost-effective outcomes.
Medical Case Management Will:
Educate the injured worker by providing them with necessary information that will help them to make informed decisions about their medical care and enable them to take an active role in treatment for their work-related injury.
Provide medical cost projections.
Identify appropriate providers and coordinate continuity of care to ensure that services are appropriate for the injury and not duplicated at any step.
Meet and consult with physicians/health care providers to facilitate timely and appropriate action.
Research treatment options in order to assist the injured worker in returning to employment in a timely manner and reaching maximum medical improvement.
Determine the need for and supply/availability of equipment.
Insure that available resources are being used in a timely and cost-effective manner.
Work with the injured worker, physicians, and therapists to establish the injured worker's physical capabilities.
Visit job sites, evaluate physical requirements of potential employment, and prepare job analyses (to include digital photos) for physician approval. We also provide video-taped job analyses, on tape or compact disc.
Work/assist physicians and employers in providing reasonable and necessary accommodations that would help in returning the injured worker to the highest possible level of employment.
Coordinate Independent Medical Evaluations.
